Description POSITION SUMMARY Provide direct patient care duties under the supervision of licensed nursing personnel and to assist in maintaining a positive physical and psychosocial environment for the patients. Essential Functions Statement(s) Assists patients with activities of daily living including bathing dressing, grooming, toileting, changing of bed linens, and positioning in and out of bed, chair, monitoring I & O, rounding on patients to ensure safety. Assists with patients' recreation programs. Prepares patients for meals and snacks, assists patient in eating where needed, and records food intake.

Reads and follows daily care plans. Reports changes in patient condition to nurse in charge. Documents care provision on patient record/flow sheets as required and reports accidents and incidents.

Provides nursing functions as directed by licensed nurse, supervisor or manager. Understands, complies with, and promotes all rules regarding patient rights. Promotes positive relationships with residents, visitors and government regulators, to include presenting a professional appearance and attitude.

Assist in admission, transfer and discharge of residents. Supports and abides by the mission, values, and policies of CHCS in all activities. On-going compliance with CHCS educational requirements.

All other duties as required. Requirements Certified Nurse Aide in the State of Kansas
