Job Description Case Manager Position Summary Under the supervision of the Case Manager Supervisor, Clinical Supervisor and Director you will be responsible to work collectively within a team environment ensuring all families' needs are met in a timely manner, following the practices and processes that support the mission, vision statements and goals of Makoon Transition Inc. Responsibilities Oversee families residing in Makoon Transition Inc., engaging them to build and meet their identified healing goals. Help them work alongside Child Family Services of reunification and assist in navigating the child welfare system.
Be prompt and ready for work in a positive manner. Sign in and out daily for accountability of attendance and tracking of time. Maintain a professional relationship with all families and other stakeholders internal and external to Makoon Transition Inc., without crossing any ethical boundaries.
Follow all rules, policies and regulations enforced by Makoon Transition Inc. and be a positive role model. Communicate any concerns or issues to the Case Manager Supervisor, Program Manager, Family Support Work Supervisor and management while following the chain of command and communications process internal and external to Makoon Transition Inc. Use strong communication skills with other units such as the Wellness team, Addictions facilitation team, FSW team within the building to ensure no gaps in services to clients and maintain strong collaboration to meet families' needs.
Apply a critical thinking, strengthâbased, traumaâinformed approach when making choices that can impact families. Be mindful of the privilege and power of choices and consider short and longâterm outcomes. Protect the integrity and work culture of Makoon Transition Inc.
Assess clientsâ physical and mental wellness needs, preferences, abilities, and develop plans to improve. Complete client assessment tools promptly while moving into Makoon Transition Inc. to ensure all needs are met and gaps identified. Work with clients, family and friend support networks, health professionals, EIA and other community stakeholders as needed to assist in advocating professionally while lowering barriers for clients.
Listen to clientsâ concerns and provide counseling or intervention as required. Complete therapy referrals for clients and children from the FNIHB/Jordan Principle list of providers. Refer and encourage clients to connect with counseling sessions at Makoon Transition Inc. with the Addictions Coordinator and Wellness Coordinators.
Encourage and empower clients to attend programming for healing and wellness. Ensure they follow all rules and agreements signed at Intake during their interview process. Schedule system meetings with clients, CFS and outside support systems to discuss strengths and areas of growth.
Be adaptable and understanding with clients, while following the mission and vision of Makoon Transition Inc. Maintain knowledge of all new and changing protocols pertaining to standards of practice at Makoon Transition Inc., drug testing protocols/practice, Child Family Services Act, policy changes within Jordanâs Principle and other information that may affect families. Record clientsâ progress through monthly progress reports which include accurate information based on shift summary documentation, wellness checks, your personal assessment of the parent and child/children.
Ensure they are submitted to the child welfare agency by the appropriate date each month. Assess clientsâ progress and encourage in a respectful and kind manner to clientsâ areas of concern. Ensure the childrenâs basic needs are met and there are no child protection or safety concerns.
Any child protection concerns must be addressed with management, the child welfare agency and/or ANCR (Child and Family All Nations Coordinated Response Network) if after hours. If a child is in danger and needs protection, communicate with all staff, increase wellness checks if needed or remove children from harmâs way if necessary. Encourage clients to reclaim their voice while allowing them their space and autonomy to choose what is best for their family.
Ensure concerns documented and/or observed by Family Support Staff and/or voiced by the client are followed up properly with clients. When doing progress reports with clients, keep the focus on children and assess/address concerns. Assess and recognize when families are ready to transition out of Makoon, communicate with the Case Manager Supervisor, Case Manager team and management to assist in creating a solid and secure transition plan.
Take part in onâcall rotation, support staff and clients when emergencies arise, which may require you to attend the building if necessary. Respond to familiesâ questions or concerns with empathy and professionalism. Assist in system navigation for clients and help lower barriers to better social, economic and health outcomes for clients.
When responding to emergency or crisis situations, contact 911 and the Management Team. Complete incident reports as needed and ensure all proper management or stakeholders are notified such as child welfare. Ensure no drugs, alcohol or illegal substances enter the building.
Provide direction and oversight of the Family Support Team Lead to ensure familiesâ needs are met. MUST have your own transportation to and from work. Drive clients on caseload as needed or if Family Support Worker is unavailable.
Do not contribute or start Lateral Violence. Participate in debriefs daily, monthly staff meetings, family events, team building activities and clinical supervision with the Case Manager Supervisor and resident meetings. Other duties as assigned.
Qualifications Grade 12 education required; college or university education in the field of expertise preferred. 3â5 years experience in Child Welfare. Valid Class 5 Driverâs Licence and access to a vehicle. CPR, First Aid and deâescalation training.
Strong verbal, written and communication skills to articulate care plans to clients, support staff and professionals. Sound critical thinking and problemâsolving skills to assess clients and analyze feedback from health care and social workers to determine the best care plans. Ability to follow the mission and vision statements of Makoon Transition Inc. for the success of the families.
Knowledge of Google Workspace and Microsoft Office with the ability to navigate shared drives. Strong organization and timeâmanagement skills to manage multiple families. Must provide criminal record, adult and child abuse registry checks.
Agree to random drug testing for illegal substance use and alcohol for work safety. MUST have your own transportation to and from work. Drive clients on caseload as needed or if Family Support Worker is unavailable.
