Gilead Community Services is seeking an attentive and recovery-oriented Case Manager to join our multi-disciplinary residential program staff. About the position: The Case Manager for Gilead's Middletown area adult residential program works Su, M, T, W, Th 3p-11p providing case management, advocacy, educational, and supportive services in a program serving adults with major mental illness and co-occurring substance use disorders. The individual in this role carries out all phases of clinical services including intake, engagement, assessment, treatment planning, case management, referral, and discharge.

This role also requires the timely completion of required documentation including assessments and other treatment planning. The compensation also includes a $1,500/year stipend added to the base pay over 26 pay periods.

Responsibilities: Providing clinical knowledge regarding severe and prolonged mental health and substance abuse disorders through training and education. Providing recovery-oriented clinical case-management services with an emphasis on dignity and respect through intervention and support, inclusion of family and community, and supportive counseling. Providing rehabilitation option services for individuals in areas including nutrition, clothing, basic care skills, spirituality, financial management, etc.

Monitoring positive outcome measures and ensuring compliance through accurate and timely data reporting. Completing assessments, treatment planning documents, and other documentation electronically and on paper in a timely manner. Acting with compassion, integrity, and ethics.
