Case Management Nurse

In this role, you will serve as a member of the Case Management Team and apply RN clinical expertise and medical appropriateness to care coordination and discharge planning.

As an RN Case Manager, you will facilitate the delivery of quality, cost-effective, patient-centered care from pre-admission through post-discharge timeframe. Base pay range $87,318.00/yr - $157,289.00/yr Key Responsibilities Coordinate with patients, families, healthcare teams, and community resources to align care goals. Create and manage care plans with clear goals, interventions, and timelines. Engage families/caregivers in planning and addressing advance directives or life-planning needs.

Monitor, document, and adjust care plans based on patient outcomes. Ensure compliance with policies, documentation, and confidentiality standards. Conduct comprehensive assessments with the interdisciplinary team, addressing medical, psychosocial, and financial needs.

Qualifications Associate’s degree in Nursing (ADN) required, Bachelor’s degree in Nursing preferred. 2 or more years of clinical experience required. 1-2 years of case management experience preferred. Active DC RN License.
