Must be comfortable working with populations with serious mental illness (SMI) and co-occurring substance use disorders (SUDs). The Case Manager will determine client needs by completing intake interviews and psychosocial evaluations and reviewing psychiatric and medical evaluations conducted by the Boston Outpatient Assisted Treatment (BOAT) team.
S/he will coordinate client support services across partnering agencies and collect data for program evaluation purposes. The Boston Outpatient Assisted Treatment (BOAT) program is a partnership between Boston Medical Center and Boston Municipal Court, to provide intensive psychiatric and co-occurring treatment to individuals with serious mental illness who are involved in the criminal justice system. The BOAT Case Manager is responsible for assessment, treatment planning, intervention with high-risk patients and their families regarding the impact/amelioration of emotional and social problems.
Must be comfortable working with populations with serious mental illness (SMI) and co-occurring substance use disorders (SUDs). The Case Manager will determine client needs by completing intake interviews and psychosocial evaluations and reviewing psychiatric and medical evaluations conducted by the BOAT team. S/he will be the liaison between the court and clinical team and will coordinate client support services across partnering agencies and collect data for program evaluation purposes.
Position: Case Manager - Boston Outpatient Assisted Treatment Program Department: Psychiatry Schedule: Full Time ESSENTIAL RESPONSIBILITIES / DUTIES: • Works within all 8 divisions of Boston Municipal Courts to provide clinical services to clients enrolled in the Boston Outpatient Assisted Treatment sessions. • Conducts intake interviews with new clients and orients them to the BOAT Program. • Determines appropriateness of referral and participation in the Program. • Works with existing partners to ensure seamless delivery of services for high need clients. • Conducts outreach to criminal justice system agencies (e.g., police, probation, court, public defender, district attorney) to educate about BOAT services and to develop working relationships. • Meets regularly with the judges and probation officers involved in the Boston Municipal Court. • Provides consultation to the Court regarding planning for mental health and substance abuse services for court-involved clients. • Communicates with Probation Department regarding oversight of client compliance with Court approved treatment recommendations. • Develops processes for clients to apply for medical insurance, housing subsidies and other entitlement programs. • Must adhere to all of BMC's RESPECT behavioral standards. JOB REQUIREMENTS: EDUCATION: Requires Master's Degree in Social Work or Mental Health Counseling • Requires level of knowledge of licensed mental health counselor processes and community resources and agencies in relation to health, illness, and disability, generally acquired through one to two years on-the-job training. • Massachusetts mental health counselor license or licensed independent clinical social work license preferred.
