Annual Compensation: $79,040 - $115,500

Registered Nurse (RN)
Position Summary
The Registered Nurse provides skilled nursing care to patients in their homes in accordance with physician orders, agency policies, and individualized plans of care. The RN collaborates with interdisciplinary team members to coordinate care, promote positive health outcomes, and support patients' rehabilitative and preventive care needs.
Key Responsibilities
- Conduct initial patient assessments and ongoing evaluations
- Develop, implement, and update individualized plans of care
- Deliver skilled nursing services based on physician orders
- Coordinate patient care with physicians and interdisciplinary team members
- Monitor patient conditions and communicate changes as needed
- Educate patients and families regarding diagnoses, treatments, and health management
- Complete accurate and timely clinical documentation, including OASIS assessments
- Participate in care conferences, quality improvement initiatives, and case management activities
- Supervise LPNs, CNAs, HHAs, and other clinical staff as appropriate
- Support preventive, restorative, and rehabilitative care services
Qualifications
- Active Colorado RN license
- Graduate of an accredited nursing program
- Minimum one (1) year of nursing experience preferred
- Home health experience preferred
- Current CPR certification
Position Summary
Licensed Practical Nurse (LPN)
The Licensed Practical Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence under the direction and supervision of the Registered Nurse. Services are provided in accordance with physician orders, the established plan of care, and agency policies and procedures.
Key Responsibilities
- Provide treatments, medications, and nursing services as assigned in the plan of care
- Observe and report changes in patient condition to the RN and physician
- Assist physicians and RNs with specialized procedures
- Prepare equipment and materials while following proper aseptic techniques
- Implement nursing care plans and preventive/rehabilitative nursing procedures
- Educate patients and families regarding home care and self-care techniques
- Maintain accurate clinical, treatment, and progress notes
- Participate in patient care conferences and interdisciplinary team discussions
- Assist in reviewing and updating patient care plans
- Accept, execute, and document physician orders in accordance with agency guidelines
- Maintain patient confidentiality and comply with all regulatory requirements
- Participate in quality assurance, peer review, and utilization review activities
- Attend staff meetings, in-services, and training programs as assigned
- Assist with on-call coverage, including evenings, weekends, and holidays as assigned
- Demonstrate sound clinical judgment, critical thinking, and effective time management
Qualifications
- Active Colorado LPN license
- Graduate of an approved practical/vocational nursing program
- Minimum one (1) year of nursing experience preferred
- Home health experience preferred
- Current CPR certification
Reporting Structure
Registered Nurse: Reports to the Clinical Manager/Nursing Supervisor.
Licensed Practical Nurse: Reports to the Clinical Manager/Nursing Supervisor and works under the direction and supervision of the Registered Nurse.
