• The candidate will be responsible for administering and documenting prescribed medications and treatments for residents while participating in the processing of medication and treatment orders according to established Lodge policies.
• The candidate will be responsible for cleaning, organizing, and restocking treatment and medication carts to ensure supplies are available for resident care.
• The candidate will be responsible for providing complete personal care to residents, including bathing, dressing, grooming, toileting, and feeding support for both able and disabled residents.
• The candidate will ensure each resident has an individualized plan of care documented through electronic software in accordance with Ministry of Long-Term Care requirements.
• The candidate will be responsible for auditing and completing resident documentation on a daily, quarterly, annual, and as-needed basis to ensure accuracy and compliance.
• The candidate will be responsible for actively participating in ongoing resident assessments using the RAI-MDS system as assigned and required.
• The candidate will be responsible for assisting able and disabled residents with transportation and support to in-Lodge appointments, meals, and activities.
• The candidate will be responsible for documenting personal care provided to residents and observing, reporting, and recording changes in their symptoms, conditions, and behaviour.
• The candidate will measure and record residents’ vital signs accurately as required.
• The candidate will receive and respond to inquiries from staff, other departments, residents, and families in a timely and professional manner.
• The candidate will monitor residents’ conditions and promptly report any accidents or incidents.
• The candidate will assist residents with transfers from beds or baths to wheelchairs using minimal physical assistance or mechanical lifts as required.
• The candidate will collect, label, and prepare resident specimens for medical check-ups and testing.
• The candidate must provide meal and nourishment services to residents while ensuring proper delivery and support.
• The candidate will be responsible for responding to resident emergencies, initiating appropriate assessments and treatments independently, and consulting with Registered Nurses as required to ensure proper post-incident care.
• The candidate will be responsible for collaborating with Registered Nurses and interdisciplinary team members to enhance care plans and maximize health outcomes for assigned residents.
• The candidate will be responsible for assisting with emergency evacuations by safely transporting able and disabled residents as required.
• The candidate will be responsible for collaborating with RNs to provide clinical direction and guidance to Personal Support Workers (PSWs).
• The candidate will be responsible for performing all duties in compliance with applicable Health and Safety legislation and City of Hamilton corporate and departmental Occupational Health and Safety policies and procedures.
• The candidate will be responsible for completing other duties as assigned that are directly related to the primary responsibilities of the position.

Job Requirements

• The candidate must possess practical nursing knowledge and skills acquired through graduation from a recognized Practical Nursing program.
• The candidate must demonstrate the ability to administer and document prescribed medications, obtain vital signs, and observe resident symptoms, conditions, and behaviours.
• The candidate must maintain current registration as a Registered Practical Nurse (RPN) with the College of Nurses of Ontario.
• The candidate must demonstrate sound knowledge and practice in accordance with the Standards of Practice governing nursing professionals in Ontario.
• The candidate must have completed the Administration of Medication course for Registered Practical Nurses.
• The candidate must demonstrate effective communication skills and the ability to establish positive relationships with peers and seniors in a Home for the Aged.
• The candidate must demonstrate proficient computer skills and knowledge of Point Click Care software.
• The candidate must demonstrate proficiency with RAI-MDS, including coding, RAPS, and RUGS processes.
• The candidate must demonstrate proficiency using eMAR (electronic Medication Administration Record) systems.
• The candidate must possess excellent written communication skills.
• The candidate must demonstrate excellent customer service skills when interacting with internal and external customers.
• The candidate must maintain a positive and professional image at all times.
• The candidate must demonstrate excellent resident assessment and care planning skills.
• The candidate must demonstrate working knowledge of Ontario Long-Term Care legislation, standards, and requirements, along with experience supporting High Intensity Needs clients and funding processes.
• The candidate with previous experience working in a long-term care environment will be preferred.
