About the Opportunity

We are seeking an experienced and compassionate Registered Nurse to join our team as a Community Dementia Nurse at Newcastle Community Health Centre. This is a unique opportunity to deliver high-quality, person-centred care to people living with dementia in their own homes. This role is both challenging and deeply rewarding, requiring strong clinical acumen and adaptability.

What you will be doing

The Community Dementia Nurse plays a vital role in supporting people living with dementia and their families by delivering safe, compassionate, and person-centred care within the community. Working closely with families and carers, you will manage a varied caseload that includes triaging local referrals, conducting telephone consultations and home visits, connecting clients with appropriate dementia support services, and providing practical education and guidance.

This role is suited to a highly emotionally intelligent Registered Nurse who understands the complexities of supporting people living with dementia at home and can respond with empathy, sound clinical judgement, and adaptability while building strong, supportive relationships with clients and their families.

You will be supported in your practice by the Nurse Manager of Aged Care Services and the Clinical Nurse Consultant (CNC), with access to the Dementia Advisory Service, Nurse Practitioner in Aged Care, and a broader multidisciplinary team. You’ll be joining a genuinely supportive and collaborative environment led by an approachable and highly experienced Nurse Manager who is invested in your success and recognises your contributions ensuring you feel valued, supported, and confident in your role from day one.

 

About You

✅ Current registration as a Registered Nurse with AHPRA
✅ Eligibility to drive in NSW and the ability and willingness to travel for work purposes which may involve long distances
✅ Experienced Registered Nurse with strong communication and listening skills, sound clinical decision-making, and the ability to rapidly assess clinical situations in the community setting
✅ Confident working independently with effective time management skills, and the ability to advocate for clients and manage a broad range of health-related needs
