About the Role
  • Provide outreach nursing care with a team in community settings for people experiencing homelessness or poverty.
  • Offer preventative care, health education, wound management, medication support, Hep C testing, and social referrals to health and community services.
  • Case management – Coordinate care and local referrals, link clients with appropriate services, and advocate for their health needs.
  • Work closely with social workers, community organisations, hospitals and other health professionals.
  • Participate in education, advocacy, and health promotion to improve access to care.
About You
  • Enrolled or Registered Nurse (AHPRA) with experience in primary health, outreach, or community care.
  • Passionate about social justice and supporting people experiencing disadvantage.
  • Able to work autonomously and problem‑solve in dynamic environments.
  • Current driver’s licence and access to a reliable vehicle (travel is required for local outreach work).
