Job Description: I. Position Summary : Reporting to the Associate Chief Nursing Officer of Hebrew Rehabilitation Center (HRC), the Social Services Manager is responsible for leading the integration and cultural transformation of the Social Services team while providing strategic and operational oversight of social services practice. This role ensures the delivery of high-quality services through effective leadership, clinical supervision, professional development, and adherence to best practices.

II. Core Competencies : Provide exemplary leadership and effective supervision to staff, fostering a culture of accountability, growth, and collaboration. Communicate clearly and professionally with staff, senior leadership, patients, and families to ensure alignment and understanding across all stakeholders.

Continuously evaluate and enhance processes and systems to support the delivery of high-quality, patient-centered care. Lead and support the ongoing integration of cultural transformation initiatives within the Social Services team, promoting engagement and sustained practice change III. Position Responsibilities : Oversees the recruitment, hiring, training, and performance evaluation of Social service staff in collaboration with the interdisciplinary healthcare team.

Provides leadership and supervision to Social service staff in collaboration with the interdisciplinary healthcare team, supporting clinical practice, professional development, and performance management. Oversees key Social Services initiatives, including advance directives, concern resolution, guardianship, admissions, and discharge planning. Serves as an active member of the interdisciplinary team, contributing to patient assessment, care planning, and culture change initiatives.

Provides clinical supervision to Social service team on LTCH units at HRC and to Social service interns. Oversees communication with patients and families, including admission orientation, ongoing updates on patient status, psychosocial support, and coordination of discharge planning as needed. Maintains a patient caseload as assigned.

Coordinates student internship programs, including academic affiliations, contracts, and compliance requirements. Supports review and approval of Social Service continuing education credits as requested. Participates in organizational committees and initiatives, including representation on the Family Advisory Council.

Ensures compliance with all applicable federal and state regulations and supports regulatory surveys and audits. Completes ongoing policy review to ensure alignment with organizational standards and best practices. Serves as a liaison and coordinator for the Family Council and leads Family Orientation sessions, while contributing to the development and implementation of the Family Education Series.

IV. Qualifications : Must hold a current LICSW license or be actively working toward obtaining LICSW licensure. Minimum of 2-5 years of supervisory experience.

Demonstrated experience in staff management, with strong interpersonal, customer service, and communication skills required. V. Physical Requirements : Ability to manage prolonged, extensive or considerable standing or walking.

Ability to manage some transporting of patients and moving of equipment (e.g., wheelchair). Ability to manage some stooping, bending and crouching. Ability to manage contact with patients under a variety of circumstances, which may include exposure to unpleasant odors, sights and sounds as well as possible exposure to infections, contagious diseases and blood borne pathogens.

Essential Job Functions Remote Type: On-site Salary Range: $77,929.00 - $140,273.00
