Case Manager CDCA

Overview DEPARTMENT/PROGRAM: Portage Area Recovery Center (PARC) REPORTS TO: Portage Area Recovery Center Program Manager SCHEDULE: (Determined on a case by case basis) General Statement of Duties Providing substance abuse related case management to clients in PARC. Responsible for overseeing residents, maintaining a therapeutic drug free environment following rules and regulations of F&CS and Ohio MHAS, and COA standards, crisis intervention, and consultation/reporting with/to supervisor(s). Responsibilities Provide crisis intervention/de-escalation and when needed consulting with supervisor.

Establish trusting relationship with clients while modeling awareness and behavior needed for growth and development of clients; displaying knowledge of the therapeutic importance of maintaining appropriate boundaries. Monitor clients use of services/treatment, and impact on independence. Provide individual and group treatment to clients.

Administer drug screens as assigned. Maintain client records in accordance with the State of Ohio regulations and F&CS policy/procedure. Participates in team planning, case reviews, and program goal achievement.

Attend agency and training meetings as requested. Fulfill other duties as assigned. Comply with agency policies and procedures, COA regulations, federal and state requirements, and educational/certification/registry requirements.
