Staff Nurse (Home Care)

• 8:30am – 5:00pm (Mon-Fri); 8:30am – 1:00pm (Sat)




• Basic salary up to $4800 per month + allowances




• Work locations: Patient homes located in the North and West area

A leading community healthcare provider is seeking an experienced Staff Nurse to join their Home Care team.

Job Responsibilities:

• Provide home-based nursing care to patients, including procedures such as tube and catheter changes, wound and stoma care, medication monitoring, and health parameter checks.




• Monitor and report patients’ conditions, educate patients and families on health management, and support discharge and post-discharge care coordination.




• Conduct nursing assessments, develop care plans, arrange home visits, and follow up to ensure community services are in place.




• Ensure safety and infection control during visits, perform equipment checks, handle visit payments and receipts, identify needs for community or financial support, and complete required administrative tasks.
  

Job Requirements:

• Diploma/ Degree in Nursing




• Possess at least 2-3 years’ experience providing inpatient nursing care




• Registered with Singapore Nursing Board




• Possess Basic Cardiac Life Support (BCLS) + AED certification
