Staff Nurse - Frailty

This post holder will work across our Elderly assessment units at both Wansbeck and North Tyneside.

The band 5 role will lead on the delivery of care assisted by a nursing assistant when caring for up to 6 patients at any one time.

They will work closely with the practitioner and medical workforce to complete required clinical assessments and investigations.

They will ensure holistic nursing assessments are carried out, and document these on dedicated IT systems.

They will co-ordinate patient admissions, discharges and transfers home and to other hospital sites.

The nurse will conduct routine unit audit and safety checks.

• To provide nursing care to a caseload of patients and to provide verbal and written handovers to nursing colleagues and the multi disciplinary team
• When responsible for the ward/dept to ensure that individualised programmes of care are formulated and carried out fully for each patient on the ward/dept (up to 30 patients in a ward area).
