Registered Nurse (RN) – Community & Home Care

Pragma Home Care is seeking compassionate, reliable and experienced Registered Nurses to join our growing community nursing team across Perth Metro and the Peel region.

The successful candidates will provide high-quality, person-centred nursing care to clients in their homes, supporting their health, independence and wellbeing.

Key Responsibilities

• Conduct nursing assessments and monitor clients’ health and wellbeing.
• Medication administration and management.
• Wound care and dressing changes.
• Catheter, continence, diabetes and chronic disease support.
• Monitor vital signs and recognise changes in clients’ conditions.
• Escalate clinical concerns appropriately.
• Provide health education and support to clients and families.
• Maintain accurate and timely clinical documentation.
• Liaise with GPs, allied health professionals and other healthcare providers.
• Follow infection control, clinical governance and workplace safety requirements.
• Current AHPRA registration as a Registered Nurse.
• Current National Police Clearance.
• Current First Aid and CPR.
• Current Australian Driver’s Licence and reliable vehicle.
• Strong clinical assessment, communication and documentation skills.
• Ability and willingness to provide community-based services across Perth Metro and Peel areas.
• Community nursing, home care, aged care or hospital experience is highly regarded.
