Role Description

This is a full-time, on-site Registered Nurse and Licensed Practical Nurse role based in Roosevelt, UT. The nurse provides direct patient care to residents, including assessments, medication administration, wound care, and monitoring changes in condition. Daily responsibilities include following individualized care plans, documenting care accurately, and communicating with physicians, interdisciplinary team members, residents, and families.

The nurse supports quality and safety initiatives, participates in care conferences, and helps train and guide nursing assistants to ensure consistent, compassionate care. The role also involves responding to emergencies, adhering to regulatory and facility policies, and promoting a respectful, healing environment for residents.

Qualifications

• Active Utah Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing; current BLS/CPR certification.
• Clinical skills in patient assessment, medication administration, wound care, and basic nursing procedures in a long-term care or similar setting.
• Knowledge of care planning, documentation standards, and regulatory guidelines related to senior care and skilled nursing facilities.
• Strong communication, interpersonal, and teamwork skills to collaborate effectively with residents, families, and interdisciplinary care teams.
• Ability to prioritize tasks, manage time efficiently, and maintain accuracy in a fast-paced clinical environment.
• Commitment to compassionate, person-centered care and respect for the diverse backgrounds and needs of residents.
• Experience in geriatric, rehabilitation, or post-acute care is preferred but not required.
• High school diploma or equivalent required; additional nursing or healthcare education and relevant certifications may be needed.
