We are seeking an experienced Primary Care Nurse Practitioner (NP) or Physician Assistant (PA) to join a unique community-based behavioral health program. The provider will deliver primary care services directly within outpatient behavioral health facilities, focusing on preventive care, chronic disease management, and improving access for underserved populations with mental health diagnoses.

Responsibilities
  • Provide primary care services to patients at contracted outpatient behavioral health facilities throughout Essex and Morris Counties.
  • Travel to one facility per day as part of a dedicated interdisciplinary care team ("pod").
  • Treat a stable outpatient population including patients with depression, anxiety, PTSD, bipolar disorder, and schizophrenia.
  • See approximately 12-18 patients per day consisting of both new and established visits.
  • Manage preventive care, chronic disease treatment, and overall health maintenance.
  • Potentially participate in light inpatient/nursing home rounding for established patients at affiliated hospitals and skilled nursing facilities.
  • Schedule is Monday-Friday, 8:00 AM-5:00 PM.
  • Telephone call coverage 1 week every 3 months.
  • Available for 1 Saturday per month, with either overtime compensation or a PTO day provided the following week.
  • Travel between sites using personal vehicle; facilities are generally located within close proximity of one another.
Requirements
  • Nurse Practitioner or Physician Assistant license in New Jersey.
  • Primary care experience strongly preferred.
  • Comfortable working with patients who have behavioral health diagnoses.
  • Spanish-speaking ability is highly preferred for the Newark-based patient population.
  • New graduates with strong training and interest in community medicine will be considered.
