Part Time Case Manager

Community Connections Inc. is looking for Case Managers to join our Day Hab team based in Wareham, MA. In this role, you will support individuals with disabilities by coordinating all services for your assigned caseload.

You will partner with Community Connections in their mission to provide optimal independence and enriching services to people with disabilities. We are looking for someone with a commitment to compassion, promoting accessibility and serving the community.
