Overview:
Helps clients and families navigate services, appointments, and transitions of care-removing barriers to access. Nurse Navigators educate, coordinate care, and advocate for culturally safe care, often in complex pathways (e.g., oncology).
What You’ll Do:
- Assess needs and coordinate care plans.
- Book referrals and support transitions.
- Provide education on conditions and services.
- Advocate for culturally safe care.
Prerequisites:
- Grade 12 diploma — Regular Grade 12, Adult 12, or GED.

Education & Qualifications:
- Nursing background preferred; strong system knowledge.
