Nephrology Advanced Practice Provider (APP) – Monroe, NY

Job Type: Full-Time
Experience: 2+ Years Required
Compensation: $120,000 Base + $5,000 Bonus (Higher Bonus Potential Based on Experience)

Position Summary

We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join a well-established nephrology practice. This role combines outpatient clinic care, inpatient rounding, and dialysis management while working closely with nephrologists and a multidisciplinary care team.

Key Responsibilities

• Provide outpatient care for CKD, hypertension, electrolyte disorders, and other renal conditions.
• Perform inpatient consultations and hospital rounding.
• Conduct daily dialysis rounds and manage acute/chronic dialysis patients.
• Order and interpret labs, imaging, and diagnostic studies.
• Develop treatment plans and educate patients and families.
• Maintain accurate medical records and participate in quality initiatives.

Qualifications

• Active New York NP or PA license and national certification.
• Valid DEA license.
• 2+ years of nephrology experience required.
• Experience in both outpatient and inpatient nephrology.
• Strong dialysis management and clinical assessment skills.
• BLS/ACLS and EMR experience preferred.
