The Geriatric Nurse Coordinator, Ambulatory Care Programs (RN) provides advanced, autonomous nursing care within an outpatient geriatrics program serving older adults with complex and often unpredictable health needs. Working within an evidence-informed framework, the role integrates comprehensive geriatric assessment, clinical triage, care coordination, and longitudinal follow-up to support optimal patient outcomes and timely access to specialized geriatric services. Applying advanced clinical judgment and assessment skills, the Geriatric Nurse Coordinator evaluates referral appropriateness, supports care pathway planning, and provides proactive case management for patients with multimorbidity, frailty, cognitive impairment, and other geriatric syndromes.

As a key member of the interdisciplinary team, the role collaborates closely with physicians, allied health professionals, primary care providers, and community partners to deliver integrated, patient- and caregiver-centered care while contributing to quality improvement and service excellence.

Skills and Qualifications Required:

  • Bachelor’s Degree in Nursing
  • Current, unrestricted registration with the College of Nurses of Ontario
  • Minimum three (3) years of clinical experience in geriatrics, care of the elderly and/or seniors health required
  • Completion of the Provincial Geriatrics Leadership Office (PGLO) Provincial Common Orientation (Geriatrics) or equivalent training required, or commitment to complete within 3-6 months of hire
  • Minimum two (2) years of experience with complex care coordination in a clinical leadership/coordinator role within a healthcare setting, ambulatory care environment preferred
  • Advanced education in Geriatrics or a related field considered an asset

Position Responsibilities:

  • Independently reviews complex incoming referrals using advanced clinical judgment and evidence-informed decision-making frameworks to determine appropriateness, urgency, and alignment with geriatric care pathways
  • Conducts comprehensive nursing assessment and clinical synthesis of referral information integrating medical, functional, cognitive, and psychosocial domains
  • Coordinates nursing and clinical support requirements for scheduled clinics through proactive review of schedules, anticipated assessment needs, and planned physician interventions
  • Anticipates patient care requirements and performs or coordinates assessments, procedures, and preparatory activities to optimize clinic flow, access, and efficient use of clinician time
  • Provides evidence-informed, individualized education and counselling, incorporating health literacy principles and adult learning strategies
  • Functions as an independent clinical resource, primarily within the outpatient geriatrics team and contributes to optimization of patient flow, access, and efficient delivery of ambulatory services
  • Identifies and communicates clinical and operational issues and contributes to development of solutions to improve patient care, access, and clinic operations
