Your responsibilities include: Assist with personal care such as bathing, grooming, dressing, and hygiene Support mobility, transfers, and range-of-motion exercises Monitor and document vital signs and report changes Provide comfort care and help prevent skin breakdown Assist with meal preparation and feeding as needed Provide medication reminders in accordance with regulations Maintain a clean, safe, and organized environment Communicate patient updates promptly to the care team Offer emotional support to patients and families Provide respite support for caregivers What You Bring: Active Certified Nursing Assistant (CNA) certification Minimum of 75 hours of CNA training One year of experience in hospice, home health, or acute care preferred Current CPR certification Valid driver's license and reliable, insured vehicle Strong communication skills and ability to work independently Compassion and professionalism when providing end-of-life care You will be successful if you are: Compassionate and emotionally aware Calm and respectful in sensitive situations Detail-oriented with strong documentation skills Reliable, accountable, and self-directed Comfortable working in patients' homes and adapting to different environments Our Culture
