Job Description
Responsibilities Act as a patient's key worker and support coordination of patient care along the treatment pathway. Ensure patients are offered holistic needs assessments at relevant points throughout the pathway. Assess patient information needs and meet those needs as required. Clinically review and assess cancer patients in the Outpatients setting. Use professional and clinical skills to act as resource and advocate for patients, promoting holistic wellbeing of patients, family and carers. Work as a core member of the multidisciplinary team, supporting the cancer pathway to improve patient experience. Deliver evidence‑based individualized patient care according to patients' changing health needs through Holistic Needs Assessment. Qualifications Understanding of the needs of cancer patients. Excellent communication skills for advising and supporting patients, relatives and other health professionals.
Knowledge of managing a range of conditions, including disease‑progression complications and side‑effect management of systemic anti‑cancer treatment and radiotherapy. Ability to speak Welsh desirable; Welsh and/or English speakers welcomed. #J-18808-Ljbffr
