The company

My client is a renowned aged care service provider and are one of Canberra’s most trusted organisations since the mid 1950’s. We are looking for a new face to join the community team and can work across the ACT area to support the elderly when required.

\n

The role:

\n

Work as a Clinical Nurse to support the community team in providing quality home care to elderly clients. You will work closely with the community nurse consultant and also support the community care management team in specialty areas of gerontology, palliative care, dementia support and mental health.

\n

Your duties:

\n

\n
• Delivering high clinical care to clients
\n
• Follow legislative requirements
\n
• Managing appointments to respond to client’s clinical support needs
\n
• Managing medical, emergency, and allied health responses
\n
• Providing clinical assessments, reviews, reports and documentation appropriate
\n
• Transitional support and discharge planning from hospital to home
\n

\n

Skills and Experience:

\n

\n
• Current AHPRA registration
\n
• Current drivers licence
\n
• Strong organisational and problem-solving skills
\n
• Minimum 2 years’ experience working with elderly clients (community work is desirable)
\n
• High knowledge of health, well-being and care support for older people
\n
