Job Description Job Description Case Manager I Housing Programs / Services At Father Joe’s Villages, access to housing and supportive services is the first step toward stability, dignity, and hope. Our Housing Programs provide safe, permanent supportive housing paired with individualized services that help individuals and families build stability and move toward long-term independence. Case Managers play a key role in supporting residents as they navigate challenges, strengthen life skills, and maintain housing, with guidance and support from an experienced team.
The Role As a Case Manager I, you play a key role in supporting individuals and families living in permanent supportive housing. The work centers on building meaningful relationships, helping clients identify and move toward personal goals, and connecting them with resources that support long-term housing stability. In partnership with a multidisciplinary team, this role includes assessing client needs, developing individualized service plans, and providing ongoing guidance and advocacy, with support and direction from supervisors and experienced team members.

Key Responsibilities Provide ongoing case management services, including assessments, service coordination, crisis support, and referrals,with guidance as needed Build supportive, trusting relationships with clients; meet regularly to review progress and respond to changing needs Develop and carry out individualized case plansin collaboration with clients and team members Conduct home visits to assess safety, well-being, and opportunities foradditionalsupport Partner with landlords and property managers to help clientsmaintainstable housing Support clients in accessing income and benefits, including employment resources, job readiness, and public benefits (e.g., Social Security, disability programs) Transport clients to appointments as needed, including medical, employment, and benefit-related services Collaborate with team members through regular meetings and case conferencing Maintainaccurate,timelydocumentation of client interactions, services, and progress Desirable Candidate Profile Bachelor’s degree in Social Work, Psychology, Sociology, Human Development, or a related field, or equivalent experience Minimumone(1)year of experience working with underserved or vulnerable populations in a social services or relatedsetting((internships, volunteer experience, or fieldwork welcome) This role requires regular driving between worksites and client locations. A valid driver’s license and the ability to meet organizational driving standards arerequired.

Work hours may vary slightly based on client needs, housing appointments, or program requirements. Compensation and Benefits Pay is based on experience, qualifications, and internal equity.
The anticipated starting pay range for this role is $22.00 to $25.30 per hour.
