Case Manager - Manager (RN / MSW) Join a mission-driven healthcare team where leadership meets impact. We are seeking a highly motivated and experienced Case Management Manager to lead our dynamic Case Management team. In this leadership role, you’ll ensure the coordination of high-quality, cost-effective patient care through efficient case review, resource management, discharge planning, and communication with third-party payers.

This is a great opportunity for a nursing or social work professional with leadership experience to make a direct impact on patient outcomes and healthcare delivery. Key Responsibilities Supervise and mentor case managers, RN case workers, and social workers Review patient charts and ensure proper documentation and levels of care Coordinate communication between departments and third-party payers Monitor and improve care transition processes and discharge planning Lead performance improvement projects and track key departmental metrics Manage department budget, operations, and staffing Develop and implement case management policies and procedures Ensure compliance with regulatory and accreditation standards Serve as second-level reviewer for discrepancies in level of care decisions Maintain current knowledge of healthcare trends, policies, and case management best practices Qualifications Education (Required): Graduate of an accredited nursing school or master’s degree in social work (MSW) or related field Education (Preferred): BSN or MSW Certification in Case Management (e.g., ACM, CCM, CPHQ) Licensure/Certifications: RN License (Oregon) OR LCSW/LMSW (Oregon) Case Management/CPHQ certification preferred (required within 2 years of hire) Experience: Minimum 2 years in case management in a hospital/healthcare setting Prior supervisory or management experience Familiarity with insurance regulations, discharge planning, and clinical documentation Skills & Traits Strong leadership and mentoring abilities Excellent communication and interpersonal skills Deep understanding of healthcare regulations, discharge planning, and payer policies Organizational and time management expertise Ability to collaborate with interdisciplinary teams and external agencies Why Join Us?
